Job Description

Coffeyville Regional Medical Center, or "CRMC", is a 25-bed, non-profit, critical access hospital serving Southeast Kansas and Northeast Oklahoma since 1949. As a rural hospital, CRMC is surprising because of its large size and wide variety of services. Although we can provide many of the same services you will find in a big city, we take great pride in our friendly and caring staff.

The Swing Bed Coordinator is responsible for coordinating the admission and discharge of Swing Bed patients, including communication between the interdisciplinary team, insurance requirements and transition between inpatient and off campus locations. In addition, h/she will act as a case manager throughout the patient’s stay, arranging additional resources, educating family, and coordinating aspects of follow up care.

JOB DUTIES

  • Serves in a marketing/recruitment capacity to other facilities to ensure awareness of CRMC Swing Bed program and works with those entities to transition patients care back to CRMC after transfer
  • Works with discharge planner and utilization review nurse to identify and transition patients from an acute status to a swing bed status as needed based on skilled nursing needs
  • Develops individualized treatment plans that account for physical, financial and other constraints, adjusting treatment protocols as needed
  • Regularly consults with nursing staff and multidisciplinary team to evaluate patient’s status and appropriateness of medical care including admission, length of stay, transfer and discharge
  • Responds to patient and family regarding questions and complaints and helps to ensure satisfaction
  • Verifies insurance coverage and monitors authorizations related to skilled benefits
  • Orients patients and families to the Swing Bed Program via the Welcome to Swing booklet and acknowledgement forms
  • Performs Swing Intake utilizing the Swing Comprehensive Assessment tool
  • Maintains compliance and knowledge regarding accreditation standards required by regulatory agencies
  • Actively works with outside entities to maximize utilization of Swing services from other referral sources
  • Serves as a resource to other staff in providing specific details and decisions regarding the Swing program
  • Maintains records and administrative reports pertaining to Swing as appropriate
  • Assists Swing patients in daily activities and promotes those activities with other staff involved in patient care

The Swing Bed Coordinator assumes other responsibilities as assigned.

JOB REQUIREMENTS:

  • Analytical ability to apply criteria and evaluate information to make decisions
  • Strong verbal and written communication skills; and excellent interpersonal skills in teaming with patients, medical staff and the public
  • Detail oriented with proven ability to multitask
  • Professional appearance and ease in developing rapport with others
  • Accurate clinical assessment skills regarding patient medical conditions
  • Computer skills including MSOffice, Outlook, Meditech, preferred
  • Willingness to cross train and cover other positions and responsibilities as needed.
  • Proficiency in Electronic Medical Record (EMR) software – Meditech, preferred
  • Experience in collecting and interpreting medical information as well as training staff

EDUCATION AND EXPERIENCE:

  • Associate Degree, Diploma or Bachelor’s Degree in nursing or licensed in Social Work, or Licensed in Physical, Speech or Occupational Therapy, preferred
  • Current licensure with the State of Kansas, required if licensed
  • One to two years’ experience in Case Management/Discharge planning, or Swing Coordination, preferred
