Position Overview
GreenLife Healthcare Staffing is seeking a dedicated Licensed Master Social Worker (LMSW) to join a review organization as a Nursing Home Surveyor/Complaint Investigator . This role involves conducting investigations and surveys at long-term care facilities to ensure compliance with Federal and State regulations and improve care quality for residents.
Key Responsibilities
- Conduct surveys and complaint investigations related to abuse, neglect, and compliance in nursing homes
- Draft detailed Statements of Deficiencies (SOD) using established documentation standards
- Assess facilities’ compliance with Plans of Correction
- Complete timely data entry into Federal and State systems
- Participate in Immediate Jeopardy situations and monitoring
- Attend internal meetings and testify in administrative hearings as required
- Adhere to Department of Health (DOH) schedules, policies, and procedures
- Travel regularly to assigned facility locations

Requirements
Qualifications
- Active New York State Licensed Master Social Worker (LMSW) license
- Minimum of 2 years clinical experience in a medical facility, preferably a nursing home
- Preferred: 2 years of experience in nursing home surveillance or complaint investigation
- SMQT certification is preferred but must be obtained within 12 months of hire
- Valid driver’s license and willingness to travel for on-site reviews
